Authorisation form - Register an entity to access Environment Online

IN THIS ARTICLE:

This page explains how to register your organisation as an entity in Environment Online and nominate a Service Administrator.

 

 

When to use this form

Complete this form if your organisation needs to:

  • create its Environment Online entity profile
  • nominate a Service Administrator
  • select a login method (Digital Identity or Microsoft Work Account).

If your organisation is already registered, you do not need this form to add or update users.

 

 

How to complete the form

Choose the relevant section based on the login method your organisation will use:

  • Form 1 — Digital Identity (myID)
  • Form 2 — Microsoft Work Account (Entra ID).

Enter your organisation’s details and nominate a Service Administrator. 

 

 

Submit the form

Email the completed form to EO Support at eosupport@dwer.wa.gov.au.

EO Support will contact you when registration is complete.
